Most advice about healthcare jobs in the US is written for people who can already work here. If you are on F-1, the question that actually decides where you apply is narrower: has this employer sponsored anyone before?
Public federal filing records answer it. Below are the healthcare, pharma and life-sciences employers with the highest counts of approved H-1B petitions in FY2023, drawn from the same data behind the employer directory.

Notice what is at the top. The biggest healthcare sponsors are insurers and pharmacy-benefit companies, not hospitals. CVS, Cigna, Elevance, Optum and Humana are, for hiring purposes, data and analytics employers that happen to work in healthcare. If your degree is in biostatistics, health informatics or data science, that is where the volume is.
The part most people miss: cap-exempt employers
Every year, cap-subject employers compete in a lottery for a limited number of H-1B slots. Registration happens in March, and if you are not selected, you wait a year.
Some employers are exempt from that cap entirely. Non-profit hospitals, universities and affiliated research institutions can file an H-1B petition at any point in the year, with no lottery. In healthcare this is unusually common, which makes it one of the few genuine structural advantages available to an international graduate.

Which roles these employers actually hire
The non-clinical side is where international graduates are hired most:

- Health Data Analyst and Health Informatics Analyst — insurers and health systems
- Regulatory Affairs Associate and Quality / Validation Engineer — device and pharma manufacturers

Clinical licensed roles (nursing, pharmacy, physician) run through separate credentialing routes and are not what these counts describe.
How to use this list
- Start with the cap-exempt employers if timing is your constraint. No lottery, and they file year-round.
- Apply to the insurers for volume. They sponsor the most, and the roles are analytical rather than clinical.
- Check the wording of the posting before you spend an hour on it. Many healthcare postings carry citizenship or clearance requirements that rule out visa holders regardless of the employer's record.
- Mention GxP or FDA exposure if you have it. 21 CFR Part 11, GCP, ISO 13485 and CAPA work are the terms pharma and device recruiters screen for.
What these numbers are not
An approval count is a record of what an employer did, not a promise about what it will do. A company that filed 300 petitions last year can still decline to sponsor the specific role you applied for. Treat the list as a way to decide where your hours are best spent, then confirm with the employer directly.
